How We Work With Your NDIS Plan

  1. Submit your onboarding form: Complete the NDIS onboarding form with your details and plan information so the team can understand your needs and funding.
  2. Initial call with the Client Services Team
    A team member will contact you to confirm your details, answer any questions, and prepare your personalised Service Agreement.
  3. Review and sign your Service Agreement
    The Service Agreement will be sent to you or your nominated representative for review and approval before sessions begin.
  4. Begin your first sessions
    The Client Services Team will book your first four sessions. The first two sessions focus on building rapport, understanding your needs, and clarifying your goals.
  5. Receive your personalised Participant Care Plan
    After your second session, your practitioner will develop a Participant Care Plan outlining recommended session frequency and how your therapy will support your goals. This plan will be shared with you for transparency and approval, and can be adjusted over time as your needs change.
  6. Ongoing sessions and support
    Once approved, the Client Services Team will schedule your remaining sessions in line with your Care Plan and your NDIS funding period, ensuring consistent and structured support.

These meetings bring together allied health professionals, therapists, and support providers three months before a plan review to proactively discuss and coordinate your supports.

By coming together as a team, we can identify the supports your family needs across the board, help ensure funds are used efficiently each quarter, save you hours of administrative work, and make the NDIS system easier to navigate and less stressful.
